Resource 1: Shared Decision-Making Flow Chart
This flow chart is designed to be used by clinicians as they work with stroke survivors and their caregivers through a 6-stage shared decision-making process to create a personalised physical activity program. (Note: we suggest printing the Shared Decision-Making Tree on A3 paper).

This detailed guide supplements the flow chart and provides more detail.
Resource 3: Infographic about Physical Activity
This infographic can be given to stroke survivors and their families when they are considering whether a physical activity program would be beneficial for them. It provides some information about physical activity, some suggested questions and links to resources from the Stroke Foundation.
Resource 4: Infographic highlighting Physical Activity Options
This resource provides some examples of physical activity options with photos of stroke survivors participating in these. This resource can be shared with stroke survivors when they are deciding what type of physical activity they would like to do. It also links to resources from the Stroke Foundation.
