Reshaping Paedeatric Venous Thromboembolism Management
The landscape of paediatric venous thromboembolism (VTE) management has been reshaped with the release of new and updated international clinical practice guidelines. Joining a global team of experts from the International Society on Thrombosis and Haemostasis (ISTH) and the American Society of Hematology (ASH), Dr Sophie Jones, from the Department of Nursing, was a key contributor, and author, in this collaborative effort. The new clinical practice guidelines capture important advancements in the field and marks the first major revision since the guidelines' initial publication in 2018.
Paediatric VTE is a rare but potentially life-threatening condition that affects children and adolescents. This disorder involves the formation of blood clots in veins and most often occurs in hospitalised children with underlying conditions, such as cancer, congenital heart disease, or those requiring central venous catheters. Although less common than in adults, paediatric VTE may result in prolonged hospitalisation, lifelong treatment, chronic pain, and long-term complications such as post-thrombotic syndrome. For children receiving treatment for VTE, this can mean disruptions to their education, social development, and overall quality of life. For families, a diagnosis of VTE can be distressing and disruptive, frequently causing emotional strain, anxiety, and the need for substantial adjustments to daily life and care routines. Early recognition, prompt treatment, and appropriate follow-up care are crucial in minimising the impact of paediatric VTE and supporting affected families through this challenging experience.
The updated VTE guidelines are a significant achievement, aiming to improve health outcomes for children with VTE through more effective and potentially safer treatment options. A key focus of the update is the upfront use of direct oral anticoagulants (DOACs) and guidance on treatment duration, marking a notable shift in treatment approaches for children with thrombosis. These guidelines provide a comprehensive framework for clinicians to make informed decisions in the management of paediatric VTE, potentially transforming the standard of care for young patients facing this challenging condition.
